Along with your completed application C&S requires the following:
1. Valid Texas Driver's License
2. Social Security card
3. Current Copy of Your Private Vehicle Insurance With Declarations Page
4. TB Test Results (Chest x-ray or PPD test)
5. Current CPR Card
6. Professional License (if applicable)
We will process your application as soon as all of the required items are received. Applications may be submitted anytime, however interviews are by appointment only.
Completing the application and having an interview with our agency does not constitute an offer of employment.
